SeniorBridge (SB) is a geriatric care management and home care company that helps people understand, manage and master the difficulties of aging. We work with people who have complex and chronic medical conditions and functional deficits. The company s unique and holistic approach addresses the total well-being of our clients and their families with the goal of keeping our clients safe and cared for in the comfort of their home. Care Managers, both Nurses and Social Workers, provide a comprehensive approach of assessment planning service coordination and advocacy. Without the type of care offered by SeniorBridge, people with chronic conditions would be placed in nursing homes, dementia units or other type of long term care institutions.
