Overview: The Director, Clinical Operations reports to the Executive Director and has an advisory reporting relationship through the CVPH Chief Nursing and Operating Officer related to regulatory compliance and scope of clinical practice. There is a collaborative reporting relationship to the Director of Practice Operations and all Practice Supervisors for matters of clinical practice, regulatory readiness, and process standardization. This position also provides functional guidance to Practice Supervisors on issues of clinical practice, and regulatory readiness. The Director of Clinical Operations is responsible for: Standardizing operational and clinical processes which support the achievement of an optimal patient encounter. Working with the Practice Leadership Team to develop and direct efficient and effective patient flow systems. Coordinate operational process across all clinical service areas. Regulatory compliance and scope of clinical practice in the outpatient practice setting. Is the primary liaison with CVPH ISS regarding the electronic health record within the outpatient practice environment and is jointly responsible for ensuring its optimal utilization. Supporting the outpatient practice by staying current on standards and providing education, training and monitoring activities to ensure a state of continual regulatory readiness. Ensuring nursing practice as well as the practice of other clinical and technical staff conforms to applicable guidelines and consistently focuses on patient safety, clinical quality, and operational standardization. Providing oversight for coordinating/administering orientation and ongoing education and competency of all staff and supervisors employed in the outpatient practice clinics. S/he understands the breadth of clinical activity that occurs across all clinical specialty and primary care areas, serving as consultant to organization Directors and Practice Manager/Coordinators supporting compliance with, licensure, credentialing, and scope of practice for all clinical staff activities. Responsibilities: Assess, design, implement and monitor systems to support the delivery of high quality, service oriented care in the outpatient practice environment. Provide leadership on clinical projects spanning all practice sites including planning and implementation of new services with guidance and inclusion of CVHN stakeholders. Ensure appropriate processes are in place to provide excellent patient service. Monitor patient, provider, and staff satisfaction levels, in collaboration with others as identified, make recommendations and implement modifications as necessary. Provide support and guidance to staff in the use of clinical care documentation and practice management systems. In conjunction with CVPH ISS EHR Manager and staff, support optimization and standardization of clinical documentation to ensure compliance with a variety of incentive and pay for performance programs including but not limited to Meaningful Use, Medical Home and Adirondack ACO. Identify appropriate benchmarks, develop clinical performance measurement tools and consistently measure clinic performance toward agreed upon targets. Work with CVHN and CVPH Directors/Leadership to enhance identified department specific processes and systems. Ensure compliance with all Joint Commission, CLIA, New York law, CMS and other agencies that regulate outpatient practice clinics. Ensure systems, policies and procedures are in place to regularly monitor and evaluate compliance with Medical Home, NCQA, PCMH, Adirondacks ACO, OSHA, CVPH and other applicable regulatory and organizational protocols and initiate improvement plans as needed. In collaboration with CVPH clinical and support services departments, develop, implement and monitor compliance with the annual plan for regulatory education, surveillance, and reporting. Initiate appropriate improvement plans as needed to ensure compliance by clinic staff and organizational leadership with the Director of Practice Operations in conjunction with CVPH HR. Ensure systems, policies, procedures and staff knowledge to facilitate participation in pay for performance programs including but not limited to Meaningful Use, Medical Home and Adirondack ACO. Provide oversight for and ensure appropriate clinical scope of practice within all clinical sites. Ensure the leadership team is kept abreast of regulatory developments, baseline performance against standards and opportunities for system-wide and department specific improvements. In conjunction with CVPH clinical laboratory, design and implement Point of Care Testing programs to support appropriate clinical practice. Responsible for the orientation and ongoing education of all appropriate clinical staff as assigned by HR and Operations Leadership. Oversee orientation, training, initial and annual competency testing and associated documentation for outpatient practice clinic staff. Ensure clinical training and competency materials are kept up to date with professional and regulatory standards. Develop and maintain all patient care practices, materials, and policies. Work collaboratively with practice leadership to ensure all are understood by all members of the outpatient practice clinics. Work with CVHN and CVPH HR leadership to ensure practice clinic staff have access to professional development courses and participate in professional career ladder programs. Responsible directly or indirectly for developing clinical staff; Nursing Structure, Medical Assistants, in cultivating skills through courses and committee involvement. Responsible for maintaining a robust practice supervisor orientation and ongoing training program to facilitate a high level of site based performance. Support, contribute, and participate in the development and implementation of CVHN General Orientation program including the HealthStream staff education system. Expert on outpatient practices and communicates across the system to ensure a collaborative environment of patient care. Serve as an advocate for excellent outpatient practice care, representing outpatient practices at applicable meetings and workgroups. Provide monthly summary to Director of Practice Operations which includes but is not limited to current practice performance, action plans for identified areas of improvement, training programs, adverse patient events and human resource hot spots. Other duties as assigned. Qualifications: BSN/MSN in Nursing or active enrollment in coursework with completion within three (3) years required. Master's preferred. Currently licensed as a Registered Nurse in NYS. Strategic thinker with demonstrated leadership skills, including abilities to direct change to meet organizational needs, express concepts clearly both verbally and written, analyze, problem-solve, prioritize quickly and efficiently and function in a team environment. Experience/skill within a training/education environment. Ability to interact with physicians, staff and other professionals. Experience and familiarity with Joint Commission CLIA, CMS and NYS regulatory environment as it pertains to ambulatory practice. Demonstrated experience and knowledge working within an electronic health record required. Working knowledge of current pay-for-performance, NCQA-PCMH and Meaningful Use protocols and requirements helpful. Minimum of 3 years demonstrated leadership experience in an ambulatory physician practice setting (either primary care and/or subspecialty) required.
