Registered Professional Nurse35Hours/Week; 52 Weeks/YearGraduate of an accredited school of nursing with a current NYS license to practice as a Registered Professional Nurse. Previous experience working with developmentally disabled and/or psychiatrically disabled individuals preferred. Reports directly to the Director of Vocational Rehabilitation Services, Assistant Director of Day Habilitation Services or Director of Day Services.GENERAL RESPONSIBILITIES:Provision of medical services, training and counseling for individuals who participate at Saratoga Bridges. Provides training and instruction in health areas for employees as requested. Assures that all programs remain in compliance with applicable local, state and federal medical regulations. Performs skilled nursing health education and other pertinent nursing services to Sheltered Workshop, Supported Employment, and Day Habilitation Service program participants.SPECIFIC RESPONSIBILITIES:1.Provides first aid to consumers and staff as necessary.2.Administers medication according to doctors orders, performs nursing assessments and provides urgent care and first aid to program participants.3. Maintains uptodate medical section of individual's case file, including but not limited to:a. monitors and charts seizure activityb. maintains medication recordsc.monitors physicalsd.monitors monthly blood pressures, blood sugars and weights as needed.e. hepatitis statusf.annual TB statusg.medication administration recordsh.nursing interventionsj. medical formularyI. Evaluation of ability to selfadminister medication.II.Medication administration record completed for each medication administered.III.Bimonthly clinical summary sent to the prescribed physician for long term services and following the dictation for training.VI. All other nursing documentation according to policiesand procedures.4.Reviews annual physicals and health records with individuals, Program Coordinators and Day Habilitation Instructors.5.Communicates with physicians concerning individuals medical issues and requesting physicians orders for these services.6. Maintains an inventory of medical supplies and orders supplies in accordance with Saratoga Bridges purchasing policies.7. Administers mantoux testing for participants and staff.8. Administers hepatitis vaccines to participants and staff.9.Tracks Hepatitis B vaccines, notifies staff of scheduled appointments, ensure completion of vaccine or declination.10. Informs program administrators and supervisors of medical issues and needs.11.Serves as member of Agency committees, including but not limited to, Safety Committee and Infection Control Committee.12.Assists program participants in scheduling medical/dental appointments.13. Attends and participates in inservice training.14.Conducts inservice training for staff on medical topics including, but not limited to, Medication Administration, CPR, First Aid and Blood Borne Pathogens and Recertification classes.15.Independently transfers consumers to and from wheelchairs, bathroom facilities and therapy tables and performs any other transfers as required.16. Fills in for other Day Habilitation Services RN vacancies in other Day Habilitation programs, as needed.17. Performs all job duties in a safe manner by observing all Agency safety rules, procedures and practices.18.
